The questionnaire for Frederiksberg

The questionnaire is an important part of the report. The questionnaire must be completed no later than one week before the first interview in the clinic.

If you have a partner, the partner must also fill in parts of the questionnaire. When you fill in your partner’s gender, the questions to your partner appear automatically.

The questionnaire is sent to the clinic via a secure connection.
