We will only process your personal data for the purposes outlined below and will always seek your explicit consent if there are any changes to our processing. If this is the case, your treating nurse or physician will inform you clearly regarding this.
That permission is not necessary if the data is used anonymously. This may be the case when using several registrations that are necessary for us to measure quality, compare test results and be held accountable to insurance providers and the government. It is not possible to trace anonymous data back to any living person.
We understand the importance of not gathering unnecessary or too many personal details and will only ask for that information which is necessary to facilitate your medical treatment at the MCK Fertility Centre and answer your questions.
Consent - where you have provided consent for us to use your information, to supply information about TFP, its services, activities, news and for marketing purposes. We will always ask for your consent before processing your personal data, and you can change or withdraw your consent at any time by contacting us using the details at the end of this policy. If you would like to withdraw your consent to which you previously opted in, you can also do so by contacting us either by telephone, post or email.
Provision of a Contract - to perform a contract with you to provide our treatment services, or supply you with requested information, for managing job application, members of staff and as contractors and sub-contractors.

For medical insurance administration, your personal data will be linked to a ‘DOT registration’ (regulated units of medical care as demanded by your medical insurance provider). This allows us to declare treatments to your medical insurance provider.
to assist tax authorities, to prevent and detect fraud, for benefits and tax administration and as part of an appeal.
to fulfil our legal obligations under the relevant fertility laws/legislation when providing fertility treatment to patients.

In your Vital Interests - We may use your personal information without your consent if we consider it in your vital interests to do so. In other words, using your information in this way will prevent or reduce any potential harm to you.
Our Legitimate interests – TFP will have a legitimate interest for using your personal information when you make contact with us or request information from us, and this will be used until you become a patient of ours, when we have a legal obligation to process your information to provide patient services.
You have the right to ask us specifically what personal details we process. To that end, you can submit a request for a copy of the data we hold on you. We will then handle your request for information as soon as possible and respond in writing in four weeks, at the latest.
When you submit such a request to us, we will ask you to provide some personal details in order to verify your request.

Your data will be stored in a scheduling and invoicing system, as well as in an electronic patient management system, which is a legal obligation, to facilitate our provision of high-quality medical treatment. In the electronic patient management system, the most pertinent medical information as well as data regarding treatments and examinations you’ve undergone will be stored. In addition to this, the electronic patient management system contains relevant data obtained from your general practitioner and/or data from (previous) treatments and examinations at other locations.
For our online events we use the service provider edudip.com. Depending on the date selected, your data for registering for the online event will be used to organise the event as well as for statistical evaluation. We may contact you by telephone to obtain feedback following the event. If you do not wish to be contacted, please let us know. Your data will not be passed on to third parties. Registration for an event is not binding and does not automatically result in a legal transaction, and you can cancel your registration for a planned event via the link provided in the confirmation email. All data you send us in the form on our website, except your personal contact details, will be published in the Guestbook, and stored for the purpose of processing your contact request. This data is only used for the unique identification of data records, for queries or for processing a request to withdraw consent, and securely stored until your withdrawal of consent.
We also use your personal data for internal meetings aimed at quality improvement and to inform you personally on process related issues in order to improve your client experience.
Periodically we will send you surveys to measure your appreciation of our services. The answers you give will be used to improve the quality of our services. We use an external third party to conduct our yearly client satisfaction survey. We agreed a Data Processing Agreement (DPA) with this external third party and the data you provide during this survey will only be used for that purpose.
You can ask the MCK Fertility Centre to transfer your personal data to another healthcare provider. We will then contact this healthcare provider and send your medical records to your new healthcare provider.
You can submit an objection by sending a letter to the abovementioned address (also see Section 16), addressed to the “Functionaris Gegevensbescherming van MC Kinderwens” (Data Protection Officer of the MCK Fertility Centre).
If you do this, the MCK Fertility Centre will not process any of your personal data, at least until there is a verdict on your objection.
We are bound by lawful retention periods for healthcare, financial, insurance and employment records etc, and your personal data will be deleted after these retention periods are over.
In addition to this, we have set timelines for how long different kinds of data can be stored by us; after which your data will be deleted.
In principle, your medical data will not be stored longer than legally obliged. Only in exceedingly exceptional cases is it allowed to store your data for longer than that.
You can ask us to amend your personal details. For example, if you would prefer to receive mail or another (digital) type of correspondence from us at another address, you can request an amendment of this data. You can also request an amendment of any other personal/demographic changes. In addition to this, you can request we correct your data if it is incorrect or incomplete. We will endeavour to immediately amend your data.
If it is unclear whether your data is allowed to be used further in the activities of our services or if your data is incomplete, you can request that we restrict its usage of your data during that time. For example, if you no longer wish to be registered as a donor.
You can request an amendment or deletion of your data by contacting our administrative office and indicating that you intend for your data to be amended or deleted:
After submitting your request, you will receive a confirmation that the we are handling your request, and the steps we will take.
In addition to exercising your legal rights as mentioned above, you can submit a complaint or request for further information to the MCK Fertility Centre’s Data Protection Officer at any time if you have a complaint regarding the way your personal data has been handled or questions regarding either this privacy statement or the way we handle your personal data.
